What do I need to book an appointment?

You will need:

1) Health care card

2) Valid credit card

3) insurance (if applicable)

What can I expect at my first appointment?

You will be required to fill out a comprehensive intake form prior to your treatment. This is necessary to make sure that we can provide the right kind of care in the safest way possible!

Once you’re at your appointment we will do a spoken intake as well as an assessment to create your unique treatment plan.

From there your job is to just relax and enjoy the treatment!

Do you do direct billing?

Short answer, Yes!

Select insurance providers will be eligble for direct billing.

We are a new practice and our billing system is still in-progress, so direct billing may not be available right away!

We appreciate your patience until everything has been processed by the insurance providers!

No Shows/Late cancellation Policy

Out of respect for the pracitioner Lōkahi Wellness has a strict no show and late cancellation policy. We require 24 hours of notice for any cancellations or changes to your appointment. Otherwise your account will be subject to a 100% charge of the appointment originally booked.

Why do you need a credit card to book an appointment?

We require a credit card on file to secure your appointment and respect the time reserved specifically for you. Your card will not be charged without your knowledge or consent, and payment is only processed in accordance with our cancellation policy. This helps us provide consistent, reliable care while ensuring appointment availability for all clients.
